Will Cambrooke Foods Bill Your Insurance Company Directly?
Cambrooke Foods is committed to making the lives of those who require low protein diets easier. Direct billing assignment can save you time, money, and monthly paper work hassles.
If you or a family member has PKU or another inherited metabolic disorder, you may be eligible to receive reimbursement for your formula and low protein food purchases. This program is intended for direct billing to insurance providers in the United States only. If a state agency or non-profit organization is interested in becoming a customer, please contact us on our toll-free line at (866) 4-LOW-PRO / (866) 456-9776.
If you are eligible to receive reimbursement under your state's legislation, we ask that you follow the procedure outlined below:
- Download Cambrooke Foods' Green Light™ Application or call us at 866 4 LOW PRO (866 456 9776) to have us fax you a complete package. (The application may take a few minutes to load -- depending on your connection speed.)
- Fill in the application completely and include copies of insurance cards as outlined in the instructions.

We Offer Direct Billing to Save You Time and Money
Educating Your Insurance Carrier
In order to meet your insurance company's requirements, you may need to supply them a copy of a Letter of Medical Necessity and/or a prescription from your doctor or healthcare provider that states why the individual needs low-protein foods. Have your doctor address this letter to your case manager.
For HMOs, you may need to obtain a written doctor's referral identifying Cambrooke Foods as the supplier of medical foods to which you are referred.
Some insurance administrators may not be familiar with the policies regarding medically prescribed foods. To help gain authorization quickly you should be armed with the facts. Cambrooke Foods recommends the following:
- Know the law in your State -- Obtain from your State's Insurance Division a copy of the legislation that mandates reimbursement for low-protein foods.
- Know the limits of your insurance Policy -- Review your insurance policy to determine if the benefit is covered, and to what extent it is covered. Be aware that self-insured companies (example: ERISA policies) are not mandated to reimburse.
- Know your Claims Manager - Connect with the right person in your insurance company that follows special claims. Be sure to get their direct dial line to avoid endless phone mail prompts.
- Call us or check our web site for updates. When available, we will post additional information. Let us know what you learn, we can all work together to make managing a special diet easier.
Once You're Approved
Cambrooke Foods will activate your account and notify you of your direct billing status. You will receive a unique customer number and can begin placing orders immediately.
Please help us provide you prompt service by notifying us of changes to your insurance coverage. This will allow your food to ship promptly when ordering. To the extent we can, Cambrooke Foods is happy to assist in this process, or to answer any further questions you might have.
